Pharmacological action

Semisynthetic broad-spectrum antibiotic of the rifamycin group. Has bacteriostatic, but in high concentrations - bactericidal action. Highly active against Mycobacterium tuberculosis, TB is the first-line drugs. It is active against Gram-positive microorganisms (Staphilococcus spp., Including multidrug resistant; Streptococcus spp., Bacillus anthracis), as well as against some Gram-negative microorganisms. Operates on pathogens Brucella spp., Legionella pneumophila, Salmonella typhi, Mycobacterium leprae, Chlamydia trachomatis. Rifadin resistance develops quickly. Cross-resistance to other anti-TB drugs (except for the remaining rifampicin) were not observed.

Pharmacokinetics

Rifadin is well absorbed from the gastrointestinal tract. The maximum concentration in blood achieved after 2-2,5 hours after ingestion. At the therapeutic level of concentration of drug ingestion is maintained for 8-12 h, with respect to highly sensitive agents - within 24 hours Rifadin penetrates well into tissues and body fluids and is found in therapeutic concentrations in pleural exudates, sputum, contents of cavities, bone. Greatest concentration of the drug created in the liver and kidneys. Because the organism is excreted in the bile and urine.

Indications

Dosage

Rifadin ingest on an empty stomach (for 1/2-1 hour before eating). Inside adults prescribe a dose of 450-900 mg / day in 1-3 reception (depending on the evidence). The maximum daily dose - 1,2 g.

Children under the age of 12 years prescribed in doses of 10-15 mg / kg body weight / day in 1-3 reception. The duration of treatment set individually.

In the treatment of tuberculosis, the average daily dose of 450 mg 1 time / day (with poor tolerability of Rifadin daily dose can be divided into 2 admission).

In acute gonorrhea prescribe oral dose of 900 mg / day or only once in 1-2 days.

Side effect

Perhaps: nausea, vomiting, diarrhea, decreased appetite, increased hepatic transaminases, bilirubin in the blood serum, headache, impaired coordination of movements, impaired vision, menstrual disorders, leukopenia, rash, eosinophilia, Quincke's edema, bronchospasm, influenza-like symptoms, herpes infection.

Rare: acute hepatic failure, hemolytic anemia, thrombocytopenic purpura (with occasional or intermittent therapy or resuming treatment after the break).

In some cases: necrosis of the kidney tubules, interstitial nephritis, acute renal failure.
